Patient Information and Responsibilities

 

Not all insurance companies cover annual physical exams and lab work associated with the exam, removal of skin tags, warts or moles, and certain other procedures. Before you have such an exam, labs, or procedure, please contact your insurance company to verify if the charges are covered under your plan. Benefits vary according to your particular plan with your insurance company.

All new patients should arrive 15 minutes prior to their scheduled appointment. Adequate time is needed for paperwork completion and insurance verification.

We make scheduled appointments for all visits. Walk-in or work-in patients will be accommodated according to the doctor's schedule.

For all established patients, we ask that you notify the front-office staff of any changes in address, phone numbers, or insurance coverage.

Medicare patients are required to complete and sign an Advanced Beneficiary Notice Form (ABN) prior to certain services. By signing this form, you agree to pay for the services in the event that Medicare determines them not to be deemed medically necessary.

If you have an emergency after hours, weekends, or holidays, please dial (919) 467-6125 and follow the instructions to reach the physician on call. Please call 911 if you feel that you have a life-threatening emergency.

Please have your pharmacy fax a refill request to (919) 467-1728 at least two days in advance of the need for a prescription refill.

All patients under the age of 18 must be accompanied by a parent or legal guardian.

The Health Insurance Portability and Accountability Act of 1996 (HIPAA) has been enacted to protect the confidentiality of the patient's health information. Therefore, we will not be able to disclose any information regarding your care here at Cary Internal Medicine to family members, friends, etc., without the patient's written consent. With this new regulation, we are limited as to the information that can be left on voice mails or answering machines.

All phone calls and voice mail messages will be returned before the close of the business day on which they were received.

If your insurance company requires a referral for you to see a specialist, please notify our office as soon as you have made the appointment at their office. We ask that sufficient time be given for us to complete the referral. If you choose to visit a specialist without a referral, you may be responsible for payment for that visit.

  Policy regarding non-participating insurance plans:
 

In the event that we do not participate with your particular insurance plan, you may still be seen by one of our doctors. Payment in full is expected at the time of service.

   
 
Payment policies:
 

We gladly accept personal checks, cash, MasterCard/Visa/Discover credit cards and debit cards.

If your check is returned by your bank for insufficient funds or for any other reason, a $30 service charge will be charged to your account. We will then ask you to pay for all future visits by cash or credit card.

We will file your insurance carrier for services provided to you in our office. You are required to present your insurance card at the time of the visit. Without a current insurance card, we will not be able to file your claims appropriately and you will be responsible for the payment of all charges. If your insurance coverage changes, please notify us at the time of your visit. We may or may not be able to refile claims and you would be responsible for payment. We suggest that you carry your insurance card with you at all times.

Your insurance company requires our office to collect co-payments, co-insurance, and deductibles at the time the service is rendered.

Self-pay patients are seen in our office. Payments for services are required at the time of the visit.

This office does not participate in any "third party billing". In the event of an accident, etc., the patient is fully responsible for payment of all charges.

Our office performs many tests, services and procedures. If you are concerned that your insurance may not cover certain services, please contact your insurance company prior to the date of service. If the service is not covered, you may be responsible for payment of the charges.

Laboratory tests are normally drawn here in our office. Some exceptions are those made by some insurance companies that require you to go to a particular laboratory. Laboratory Corporation of America performs the majority of our tests. There may be some tests performed that Labcorp will bill you for, but the bulk of testing is billed by our office.

   
  Policy regarding copying of medical records:
 

Medical records are copied by Smart Corporation. They bill you for the cost of the service directly.
